After Booking

Once your appointment is scheduled, please complete the required intake forms prior to your visit. These forms help your provider better understand your needs and prepare for your evaluation. All forms will be sent via email along with instruction on how to access your patient portal.

Required forms include:

• Professional Liability Document
• GAD-7 Anxiety Assessment
• Patient Health Questionnaire (PHQ-9)

Completing these forms in advance allows us to provide a more efficient and personalized evaluation during your appointment.
